Healthy Hearts Group Clinic at Shepherds Spring Medical Centre

We are offering a 3-week group clinic which will support patients who are at moderate risk of cardiovascular disease (e.g., heart attack or stroke) to improve their health and well-being. We know that by making small improvements in our diet, physical activity, stress management etc. we can improve our health and reduce the risk of long-term conditions. We also know that this is difficult to do alone! By having a supportive group of other patients who are experiencing similar health concerns, and significantly longer with your clinicians to ask the questions that are important to you, this programme will support you to improve your health. It will also be an opportunity to talk about any medications that may be helpful for you and ensure you are aware of all the help that is on offer to improve your health.

When are the sessions?

Weekly for 3 weeks starting Monday 2nd, 9th, 16th October 6.30-7.30pm @ Shepherds Spring Medical Centre. By booking you confirm you will be able to attend all three sessions.

Who is running the session?

The session is being run by Dr Ruth Dyson (GP) and Sister Janet Walker (Practice Nurse)

Who can attend?

We will invite eligible patients via text message. The group is for patients aged between 18-70 years who have had a high BP reading (but are not on treatment) and for those who are at moderate risk for cardiovascular disease (e.g. heart attack or stroke).

What are the aims?

Each session will focus on an aspect of your health e.g., diet, physical activity, stress management. Through group discussion and the opportunity for a one-to-one consultation within the supportive group environment you will learn more about how to improve your health and well-being. You will also be able to discuss medication, where appropriate. The overall aim is to support you to reduce your blood pressure and/or your risk of cardiovascular disease (e.g., heart attack or stroke), and to improve your health and well-being.

What are the benefits?

The group clinic approach means the sessions are longer than your usual GP or nurse appointments (60 mins compared to the usual 10-20mins) so you get plenty of time to talk through your concerns. The group also allows you to learn from other patients who are affected by similar conditions.
